The Pupil Personal Accident Insurance is in place through Allianz Insurance.
In the event, that you need to make a claim please refer to www.allianz.ie to download and print a claim form or phone 01-6133900 to have a form forwarded.
The claim form must be completed in full by parent/guardian and then the school verifies that the claimant is on cover.
Please do not ask school to stamp a blank form, this cannot be done.
